How to submit a request
1. Complete and print the OPUS card request form, registered with photo ID
2. Go to the Personalized OPUS card service centre

You must have these with you:
- The form you filled out online, printed and signed. If you are under the age of 14, a parent.
- Your proof of age
- Your proof of school attendance
- Your proof of residence (for those 18 to 25)
- $13.50 in cash ($3.50 fee to issue an OPUS card and $10 fee for fare privilege)

3. Fill it up!
Once you have your OPUS card on hand, all you have to do is load it with valid transit fares. To find out where you can charge fares onto your card, consult the list of available points of sale.
